Panelists discussed how they utilized the CtLC framework to develop good plans that lead to good lives for people receiving, and people waiting for, paid services. They also share how they used the principles to develop and organize data and reports to support program planning and decision making related to person centered outcomes. Using CtLC to Track Success to the Good Life

In this Friday Feature, Heather Lytle from FACT shows how they used the Charting the Lifecourse domains in order to create a survey that would be completed every three months for three years. The survey would collect data that would indicate a family’s progression. The goal for all families is to be empowered to handle the challenges that come with having a child with a disability. This study supports having a Family Support Partner significantly increases a parents’ ability to overcome barriers, learn about resources and advocacy.

How does the use of the LifeCourse Tools compare with other person-centered planning tools or processes? Jim Schu and Rachel Bernini of www.sam-inc.org were interested in determining what, if any, connection existed between the use of the tools and outcome development. They highlight what they learned when comparing individualized outcomes utilizing the tools in this presentation.

Lisa Meyer is the August awardee for Innovation in Person Centered Planning. Lisa is the perfect example of a LifeCourse “super-participant”. When we first met Lisa years ago, her enthusiasm and energy for the LifeCourse framework was evident. Lisa expressed that it validated her values and the work she was doing in Pennsylvania at that time, and it beautifully complemented her role as a Mentor-Trainer in the Learning Community for Person Centered Practices (TLCPCP). Lisa has put her passion to work with the Nexus as the main cheerleader for Ambassador coaches and works to develop skilled coaches that are hosting the Ambassador Series. We are honored to continue to learn from Lisa and have her as part of the Nexus team!
